Aged Care Services
Most people, as they get older, prefer to live as independently as possible in their own homes.
Communify delivers aged care services and activities that keep older people healthy, connected and independent at home. We can provide the assistance that will make living at home easier, safer and more comfortable.
Communify is a member of healthy@home, a consortium of 18 organisations who specialise in providing high-quality community aged care services to support people with diverse needs across the Brisbane North and Caboolture regions.
We deliver Commonwealth Home Support Program (CHSP) services, funded by the Australian Government Department of Health, to help people aged 65 years and over, or aged 50 years and over for Aboriginal and Torres Strait Islander people. We aim to avoid or delay entry into residential aged care for the older people we support by enhancing their independence at home.
As a provider of in-home care and flexible aged care services, Communify is obligated to report under the Serious Incident Response Scheme (SIRS) to ensure we reduce the risk of abuse and neglect for people receiving aged care. For more information, please refer to this fact sheet.
Home Care Packages
The Australian Government Home Care Packages Program provides tailored, coordinated supports to people over 65 with complex care needs related to ageing. Communify is an approved provider of Level One, Two, Three and Four Home Care Packages.
Communify Navigators
Communify Navigators is a care finder service for vulnerable older people that Communify delivers in a consortium with MICAH Projects and Queensland Positive People (QPP). The program is part of a national network of care finder services.
Communify Navigators provides specialist and intensive assistance to help participants understand and access aged care and connect with other relevant support in the community.
Domestic assistance and in-home support
A little help around the home can make a big difference. Communify’s in-home support workers can provide subsidised domestic assistance five days a week, Monday to Friday, to eligible people.
Our in-home support workers will work with you to determine what your priorities are and what you need assistance with. We can provide a wide range of services to make living at home easier.
Home Assist Secure
Our Home Assist Secure service provides subsidised assistance with minor home maintenance or modifications to those eligible under the Commonwealth Home Support Program (CHSP) or the Queensland Government-funded Home Assist Program.
Home Assist Secure supports are designed to help improve access to and within your home, and to help you feel safer living at home.
Nursing and allied health care
We provide in-home nursing services to help eligible over-65s to remain comfortable in their own homes for as long as possible. Our friendly team of staff nurses visit you at home to assess your needs and provide nursing and personal care. They ensure you receive care at home, without having to frequently visit your GP or hospital for treatment.
We can also connect older people with allied health supports to keep them supported and active in their community.
The Paddington Centre
The Paddington Day Respite Centre provides social interaction and connection for people over 65. The centre offers a welcoming atmosphere for older people to enjoy a day away from home, which gives carers a break too, and to take part in our diverse range of activities.
Meals on Wheels
We provide in-home nursing services to help eligible over-65s to remain comfortable in their own homes for as long as possible. Transport
If you need help to get out and about because of your age, ill-health or a disability, Communify’s subsidised transport services are here for you to make transport easier and more affordable.
