Support at Home

The Australian Government Support at Home program provides tailored, coordinated supports to people over 65 with complex care needs related to ageing. Communify is an approved provider of Level 1-8 Support at Home services, delivered with the aim of increasing your independence in your own home.

Who we help

To receive Support at Home services through us, you must be:

You must also have already received approval for a Level 1, 2, 3 or 4 (Home Care Package) or Level 1 – 8 (Support at Home) through MyAgedCare.

How we help

Your Support at Home Facilitator will guide you through the process of activating your package. They will then support you to maximize your package allocation to best suit your individual needs, and continue to facilitate your independence whilst living at home.

Once you have received approval for Support at Home services, please notify us and we can organise an information session.
