Home Care Packages
The Australian Government Home Care Packages Program provides tailored, coordinated supports to people over 65 with complex care needs related to ageing. Communify is an approved provider of Level One, Two, Three and Four Home Care Packages, delivered with the aim of increasing your independence in your own home.
Who we help
To receive Home Care Packages services through us, you must be:
- Aged 65 or over
- Located in the Brisbane North PHN catchment area
You must also have already received approval for a Level 1, 2, 3 or 4 Home Care Package through MyAgedCare.
How we help
Your Home Care Package Facilitator will guide you through the process of activating your package. They will then support you to maximize your package allocation to best suit your individual needs, and continue to facilitate your independence whilst living at home.
Examples of services we deliver through Home Care Packages include:
- Nursing
- Allied Health
- Domestic assistance
- Personal care
- Individual and group social support
- In-home and centre-based respite services
- Transport
- Meals
- Purchase of equipment

To obtain approval for a Home Care Package, please visit the MyAgedCare website to check your eligibility and apply for an assessment of your needs.
Once you have received approval for your Home Care Package, please notify us and we can organise an information session.
