HART4000 (Homelessness Assessment and Referral Team) is a leading provider of housing support services in Brisbane. Based in Fortitude Valley, the HART4000 team works closely with those who may be experiencing homelessness or at risk of becoming homeless.
We believe that housing is a basic human right and we aim to support people to secure safe, affordable and sustainable housing.
While we do not have our own accommodation, we work proactively with each person to find the housing solution that is most suitable for you.
- Visit our drop-in service: Available Monday-Friday from 1pm-3pm for those who cannot wait for an appointment; however, please be prepared to wait as our drop-in service is very busy. The HART4000 office is located on Level 1, 5 Green Square Close, Fortitude Valley (Cnr Constance and Alfred Street).
- Email us: Our team is available Monday-Friday from 9am-5pm. If you require an immediate assessment of your needs, please email info@hart4000.org.au to book an appointment. We offer appointments in the morning, though these are often booked out weeks in advance.
If you are in crisis after-hours, please call the Homeless Hotline on 1800 474 753. In cases of emergencies, if life is in immediate danger, please call 000.
For Practitioners and Referral Services:
HART4000 assists anyone living in Brisbane aged 18 or over who is experiencing homelessness or at risk of homelessness.
You may be experiencing homelessness or at risk of homelessness if you are:
- Sleeping rough
- Staying with friends, family or “couch surfing”
- Sleeping in a car, caravan park or low-cost hotel
- Staying in other temporary accommodation
- Staying in emergency accommodation or a refuge
- At risk of being evicted due to rent arrears, mortgage stress or other factors
We support individuals, couples and families. If a person is under 25, we may recommend a youth service instead.
If you are located outside of Brisbane and there is no suitable service in your area that you can access, you may also be eligible to seek support from HART4000.
How we help
We offer a personalised response to your housing needs that may include the following:
- Finding sustainable housing that is suitable for you
- Housing advocacy and support
- Service coordination and planned support to help achieve your housing goals
- Direct referrals to emergency accommodation providers
- Referrals and assistance with community and social housing providers including applications, support letters, bond loan or rental grants
- TICA checks
- Information to help you navigate the housing system
Stories from our clients
Shona is among an alarmingly growing demographic now facing homelessness who have never experienced it before this recent housing crisis. A retired nurse and mother at 67 years of age, Shona had spent her life caring for others and never reaching out for help in return. In February 2023, she was living alone, privately renting an apartment in Kangaroo Point, when her landlord decided to sell the unit and allowed Shona only four weeks to vacate the property.
The 2021 Brisbane floods brought devastation to the lives and homes of so many people, and the most vulnerable members of our community in particular are still recovering. The HART4000 team had been working to secure more affordable housing for Kylie-Maree, who was living in a granny flat and spending her entire income on rent, when the weather crisis hit and completely flooded her home.
Courageous couple Shannyn and Wayne self-referred to our HART4000 Homelessness Service after enduring six months of homelessness.
